How to create a qualitative feedback form in evaluation template?

While creating the evaluation template, there is an option to create a qualitative feedback form that can be filled out by the employees during their performance assessment process and can be reviewed by their assessors.

These are the steps to create the qualitative feedback form:

• Go to Performance<<Settings<<Evaluation Template
• Click on "+Add Evaluation Template"
• Go to step 3 of the template to create the Qualitative feedback form.
• Create questions by selecting the type of question (Text, Checkbox, Dropdown, Number) and form questions.
